Senior Transportation Options in Massachusetts: What Families Need to Know

For adult children managing the care of an aging parent in Massachusetts, transportation is often one of the most complicated pieces of the puzzle. Your parent may no longer drive safely — whether due to vision changes, cognitive decline, or physical limitations — but still needs to reach doctors, specialists, lab work, and physical therapy. What are the options, and how do you choose the right one?

Council on Aging (COA) Van Programs

Most Massachusetts cities and towns have a Council on Aging that operates some form of senior van program. COA programs are typically free or low-cost and provide a valuable first layer of transportation. However, they have real limitations: most operate only within a limited geographic area, Boston hospital trips are rarely covered, vans are often booked two to four weeks in advance, and door-to-door driver assistance varies widely. For simple local trips — a nearby primary care office, a local pharmacy — COA programs are excellent. For Boston hospital visits or appointments requiring real driver assistance, they are rarely sufficient.

THE RIDE (MBTA Paratransit)

The MBTA offers THE RIDE for eligible individuals with disabilities who cannot use fixed-route transit. Available within 3/4 mile of an MBTA route, it requires ADA eligibility certification and 1–2 day advance booking. Shared rides extend trip times significantly. For suburban seniors in communities like Lexington, Concord, Needham, or Andover, coverage is limited and rarely practical for Boston hospital trips.

Private Non-Emergency Medical Transportation

Private senior transportation from a professional NEMT provider offers the most comprehensive service: door-to-door pickup from your parent's home or senior community, patient unhurried driver assistance, escort to appointment entrance, full driver wait time, ADA-compliant vehicles when needed, and drivers trained specifically for senior transport. More expensive than public programs, but also the most appropriate for seniors with significant mobility needs, cognitive changes, or complex medical schedules. When the alternative is a family member taking time off work for every appointment, professional NEMT often becomes the most practical choice as well.

When Professional NEMT Is the Right Choice

Professional senior transportation makes the most sense when your parent uses a wheelchair, walker, or has significant balance issues requiring hands-on assistance. Also when your parent has cognitive changes and needs a patient familiar driver. When appointments are at major Boston hospitals where navigation is complex. When your parent has a recurring treatment schedule. And when you live far from your parent and cannot accompany them to every appointment.
